Student Waiver and Release of Liability

User Agreement

I understand that there are specific risks of physical or property damages, losses, or injury that may result from my or my child’s participation in dance instruction by Charmaine Ristow, and I voluntarily assume the risks associated with such participation, guaranteeing the registered students to be in sound physical condition with physician’s approval. 

Further, I hereby indemnify, release and hold harmless Charmaine Ristow from any and all liability, claims, demands, and causes of action whatsoever, arising out of or related to any loss, damage, or injury, including death, that may be sustained by the participant while receiving instruction virtually by Charmaine Ristow.
